Family History Questionnaire

I ran across Amy’s blog Why Amy Smiles. She had a wonderful post about filling out a child’s interview annually to record their changes in likes and interests.

I thought it had significant value as a genealogical document that records a snapshot of the person or family at that time and place and significant events from that year. So using Amy’s idea, I created a version for adults with genealogy in mind. If these are filled out on an annual basis, your family history is practically written.
